Build your veterinary visit question list

Select questions to create a list you can print. Bring relevant records and observations; this tool organizes the conversation without assessing your dog’s health.

Start with the reason for the visit

Choose questions that match what you need to understand. For a routine review, ask about prevention and follow-up. For an observed change, prepare its onset, progression and circumstances. In an emergency, contact veterinary care without waiting to complete this list.

Bring information that can be checked

Gather reports, current medication and supplements, food information and previous instructions. Separate your own observations from information another person reported. If a record is missing, say so rather than guessing.

Keep the list before leaving

Selections remain in the open page and reset when you leave or reload. The tool does not ask for medical details or send selections as analytics parameters. Print the page to keep your list. The example below is illustrative, without recommendations for an individual dog.
